Hourly Work Authorization

By signing below, I authorize Azurite Marketing Group ("Provider") to perform website or marketing services as per the agreed upon amount, beginning on the date of this submission.

I understand and agree to the following:

1. Services will be provided at our hourly rate based on the estimate provided.
2. My credit card or payment method will be authorized (not charged) for the agreed-upon amount. The card on file will be billed at the completion of the agreed-upon work scope.
3. Any work beyond the scope of the plan will be quoted separately and requires separate approval before work begins.
