LinkedIn Maintenance Services Authorization

By signing below, I authorize Azurite Marketing Group ("Provider") to perform ongoing LinkedIn monthly website maintenance services, beginning on the date of this submission.

I understand and agree to the following:

1. Services will be provided on a month-to-month basis.
2. My credit card or payment method will be authorized (not charged) for the agreed-upon monthly amount. The card on file will be billed at the end of each month for the duration of this agreement.
3. Maintenance services include only the items outlined in the selected plan. Any work beyond the scope of the plan will be quoted separately and requires separate approval before work begins.
4. Either party may cancel this agreement with 30 days written notice prior to the next billing cycle. No refunds will be issued for the current billing period.
